The Holistic Mood Assessment starts with a thorough review of your mood symptoms through a series of detailed questions geared toward identifying specific causes, as well as a review of your diet, lifestyle, medications, general health, and more.  Read full details here

After you finish the detailed intake, you will receive a 25+ page assessment of all of the key biological areas that are likely to be affecting your mood, mental health, cognition, and overall wellbeing.

Instructions to complete the questionnaire:

1.  To enter the questionnaire, you must purchase a license code.
2. You will receive a license code and a PDF coupon for $25 off any lab kit in the Mind Body Map store in a separate confirmation email.
3.  To redeem, follow this link to the questionnaire.
4.  Enter license key as the access code from your confirmation email.
5.  Fill in your information on the first page, including, name and email address.  
6. Continue to questionnaire.  The questionnaire will take 30-40 minutes.  It is best to complete in one sitting.  If you plan to complete the questionnaire in one sitting, you may use a computer or mobile device.
7. Re-entering the questionnaire:  If you don’t think you can complete the questionnaire in one sitting, you must use a regular computer, such as a desktop or laptop, NOT a mobile device, to have the option to save your answers and continue at a later time. ***To access your custom URL to re-enter the questionnaire, you must register with your email address on the first question, and then reopen the questionnaire using the following format:  https://su.vc/moodmindbodymap/?rid=EM_youremail.  Please replace “youremail” with the email address you registered on the first page of the questionnaire.    

Downloading your report:

1. When you reach the end of the questionnaire, there will be a download link on the final page retrieve your custom report.  You will also receive an email with access to your custom report.
2. Please note that you can only generate the report one time per license code, but you will retain access to download copies of that report through the link sent to your email.
