Consultations:
Mind Body Map seeks to provide Holistic Psychiatry consultations to clients all over the U.S. While we are licensed medical providers in our home states, services through Mind Body Map will be considered health coaching and consultations due to the nature of medical licensing by state. This is not a primary care service, nor an emergency service. Patients are recommended to have a local provider in your home state for emergencies, medication management, and exams. No doctor-patient relationship will be established during the consultation.
Consultation Tech Options:
Appointments are available by phone or video conferencing through our secure video portal using Zoom software. If you elect to use the video conferencing service, you are responsible for having the software downloaded, your personal equipment working (microphone, webcam, and speakers/headphones), and having reliable internet for the duration of the appointment. If technical difficulties or user error last for more than 5 minutes, we will call you to conduct your appointment by phone. We are not responsible for appointment time lost due to technical difficulties, equipment failure, wrong phone numbers, or poor internet or phone connections, unless it is our own technology error, in which case we will reschedule or refund the fee.
Medication Management and Treatment:
We do not provide prescription medication during our consultations except in rare cases for Washington State residents only. If it is determined that you need ongoing care with prescription psychiatric medication, we will need to refer you to a psychiatrist for medication management, but will be happy to work integratively to continue to provide natural treatment. We may recommend diet and lifestyle changes, natural medicines, supplements, physical medicine, counseling, botanical medicine, hormones, or other therapies. Prescriptions for hormones, thyroid medication, etc are only available for Washington State residents.
Client Portal:
The portal is our primary communication tool where we provide reports, messaging, lab results and billing via a secure HIPAA Compliant system. Please be sure to sign up for the client portal as we will not share health information via non-secure systems such as email.
Insurance and Financial Policies:
All fees are due upon booking of appointments. We do not accept insurance. Superbills can be generated upon request for Washington State residents. Consultation services for non-Washington State residents do not qualify for insurance reimbursement, so we do not provide superbills for those consultations. If you are trying to use a Health Savings Account for services, please let us know so we can provide you with the type of report that will be required for reimbursement.
Cancellation, Rescheduling and Refunds:
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another client from getting the care they need. Please give 48 business hours notice of cancellation or rescheduling. This gives us time to offer your appointment spot to another client. No refunds are available if you do not show up at your scheduled time and have not contacted us 48 hours prior to your appointment time.
Communications, Email, and Messaging Policy:
Please bring all of your questions to your scheduled appointment. Appropriate use of follow up emails is limited to clarifying the recommendations from your last appointment when needed. We are unable to do email consultations between appointments to provide services, evaluate new or changing symptoms, make recommendations about whether to keep taking something, interpret new labs, or anything else that requires physician time. If you make an email request for something that requires appointment time, we will ask you to schedule, or to pre-pay a follow-up appointment for the time it takes to respond to your request.
Privacy:
We will not disclose your information to others unless you direct us to do so or applicable laws authorize or compel us to do so in the event that you become a risk for harm to yourself or others.
